VENDOR APPLICATION
Wiles Chiropractic would like to invite you to participate in the 2nd Annual Healthy Living EXPO on Saturday April 24th,
2010 from 10:00am to 4:00pm.  The Expo will take place at Marlboro Elementary School, located at 1380 Rt. 9W in
Marlboro, NY.  The purpose of the Healthy Living EXPO is to increase health awareness through education and
prevention.  Last year's event was a great success and this year promises to be even bigger, including the first Healthy
Living EXPO 3-ON-3 Basketball Tournament.

If you have expertise you would like to share with those attending the Health EXPO, please indicate on the response form
below.  This year there will again be NO CHARGE to participate!  To facilitate our planning, please complete the vendor
